Capture Lead Source on Sparkforge Forms and Surveys
Add a hidden Source field to any Sparkforge form or survey to tag submissions with a fixed lead source, or override it on the fly with a ?source= URL parameter to track campaigns.
The Source field tells you where a lead actually came from. Drop it on every form, set a default value, and override it per campaign with a URL parameter. The result: every contact in your CRM has a Contact Source attached the moment they submit.
Add the Source field
- Open the form or survey in the builder.
- Find Source under Quick Add.
- Drag and drop it into the form.
- Click the field, then enter a default value in the Source setting (for example,
website-contact-page).
The field is hidden — submitters never see it. The value rides along with the submission.
How submissions look
After a form submission, the captured source value appears in the submission record and on the contact record as Contact Source. Filter contacts by source to see exactly which channels are producing leads.
Override with a URL parameter
Add ?source=value to the form's link to override whatever default you set in the builder. Useful for paid campaigns, partner referrals, and email pushes.

That submission will be tagged as facebook-ad-spring-promo instead of the form's default. Use a unique source string per campaign so reporting is unambiguous.
Practical patterns
- Per-channel landing pages — Same form, different URL with
?source=baked in. - Email link tracking — Append
?source=newsletter-2026-04to know exactly which send drove the lead. - Partner attribution — Hand each partner their own link with their slug as the source.
