How to Use SMS Verification on Sparkforge Forms and Surveys

By Sparkforge Support Team ·Last reviewed May 3, 2026

SMS Verification adds a Verify Phone button next to the phone field in Sparkforge forms and surveys. Submitters get a one-time code via SMS and verified records are flagged as Phone Verified = Yes.

SMS Verification confirms that a phone number entered into a Sparkforge form or survey actually belongs to the person filling it out. Verified submissions are tagged so you can filter and act on real leads.

What you get

  • Verify Phone button: appears next to the phone field once a valid number is entered.
  • One-time code: the submitter receives a code via SMS and types it into the form to confirm.
  • Phone Verified field: form and survey submissions show a Phone Verified field set to Yes when verification succeeds.
  • Cleaner data: drops bot and typo submissions before they hit your CRM.

Billing

OTP messages are billed at standard SMS pricing for your account.

How the flow works

  1. The submitter enters a phone number in the form or survey.
  2. Once the number passes basic validation, the Verify Phone button becomes available.
  3. Clicking the button sends a verification code via SMS.
  4. The submitter types the code back into the form to complete verification.
  5. On success, the submission is recorded with Phone Verified = Yes.

When to use it

Turn it on for forms and surveys where phone accuracy matters: lead capture into a sales pipeline, appointment booking, support intake, and any place spam submissions waste team time.
