Create a Facebook Lead Form in Sparkforge Ad Manager
Create a Facebook lead form inside Sparkforge Ad Manager from the campaign builder. Choose between More Volume and Higher Intent, add questions, map fields to the CRM, and reuse forms with the duplicate option.
Lead Generation campaigns in Sparkforge Ad Manager use a Facebook lead form to collect contact information. You can create new forms, map their fields to your CRM, and duplicate existing forms without leaving the campaign builder.
Create a new Facebook lead form
- Inside the Ad Campaign builder, set the objective to Lead Generation.
- Open the lead form dropdown and click Create Facebook Form.
- Name the form.
- Pick a form type:
- More Volume: shorter flow, more submissions, lower intent.
- Higher Intent: adds a confirmation review step before submit.
- Add a Greetings section with a required headline plus an optional paragraph or list.
- Add Questions:
- Use the description to set context.
- Pick the prebuilt fields you want (name, email, phone, etc).
- Add custom questions with short answers or multiple choice for qualifying data.
- Set up Form Field Mapping so each form field syncs to a Sparkforge contact field on submission.
- Add a Privacy Policy URL. Use your own policy or copy the Sparkforge policy at
https://www.sparkforgehq.com/privacy-policy. - Configure the Thank You message: headline, description, and call-to-action like a website link or phone call.
- Save the form. It is now available for the Lead Generation objective.
Custom fields in Ad Manager
Custom fields you add to Sparkforge appear in the form field mapping section. Use them when:
- You collect data Meta does not provide as a default field.
- You need form responses to land on a specific custom contact field.
- You want to capture tracking parameters from Meta forms (these only sync after mapping).
Create the custom field in Sparkforge first, then map it inside the form builder.
Review and map an existing form
Lead forms originally created on Facebook can be reviewed and mapped from Ad Manager.
- In the Lead Generation campaign setup, select the existing form.
- Click the pencil/edit icon next to the form name (hover label: Map Fields / Duplicate Form).
- In the modal you can:
- Preview the form's content.
- Map or update each form field to a CRM field.
- Confirm lead syncing is complete.
Field mapping prompt
If a selected form has not been mapped, Ad Manager shows a prompt below the form selector: Form fields of the selected form are not mapped, map now. Click map now to open the review modal.
Duplicate a form
Duplicating saves time when you need a variant for testing or a new offer.
- Open the form's edit modal.
- Click Duplicate Form at the bottom left.
- Edit the prefilled fields as needed.
- Click Create Form to save it as a new lead form.
Best practices
- Map every form field before launching, otherwise leads come in with unsynced data.
- Use Higher Intent for offers that need quality over volume.
- Keep the privacy policy current and hosted on a stable URL.
- Duplicate rather than rebuild when iterating — mapping carries over and saves rework.
