View Form Submissions in the Sparkforge Appointment Modal
Custom form submissions tied to a calendar booking now appear inside the Sparkforge Appointment Modal under a Forms tab. No need to jump to the Forms module or contact record.
The Appointment Modal now includes a Forms tab that displays custom form submissions attached to a booking. Open it anywhere the Appointment Modal opens — Calendars, Contacts, or Conversations — and review responses without context-switching.
What the Forms tab does
When an appointment is booked through a calendar that uses a custom form, Sparkforge surfaces the responses directly in the modal. The layout mirrors the Forms module so the data is easy to scan during prep.
How to open form submissions
- Go to Calendars, Contacts, or Conversations.
- Click an appointment to open the Appointment Modal.
- If the appointment includes a custom form, a Forms tab appears.
- Click Forms to view the structured submission.
Which forms are supported
Supported
- Forms built in the Sparkforge Forms Builder.
- Forms attached to a calendar through the calendar’s Form settings.
Not yet supported
- The default booking form (Name, Email, Phone). Those fields are already shown in the Contact Details section of the modal. Surfacing them in the Forms tab is on the roadmap.
When this saves the most time
- Agencies reading discovery forms before a strategy call.
- Health and wellness providers reviewing patient intake before a visit.
- Sales reps qualifying leads from custom intake fields.
- Coaches and consultants tailoring a session from client-submitted answers.

Where does the form data come from?
From the form attached to the calendar booking or added during the appointment workflow.
Will default booking forms get a Forms tab?
