View Custom Form Submissions in the Sparkforge Appointment Modal

By Sparkforge Support Team ·Last reviewed May 3, 2026

Open any appointment from Calendars, Contacts, or Conversations. If a custom form was attached at booking, the Forms tab shows the structured response inside the modal.

The Appointment Modal now shows custom booking form submissions in a new Forms tab. Read intake answers without leaving the appointment view.

What's in the Appointment Modal

  • Forms tab: appears in the Appointment Modal whenever a custom form was attached to the booking.
  • Structured layout: responses display in a clean format that mirrors the Forms module.
  • Available everywhere appointments are: Calendars, Contacts, and Conversations all surface the Forms tab on the same modal.

How to view a submission

  1. Open Calendars and click any recent appointment.
  2. Open the appointment modal by clicking the appointment card.
  3. Look for the Forms tab. If a custom form was submitted during booking, the tab shows in the modal.
  4. Click Forms to read the structured response.

Important notes

  • Supports custom forms added to the booking flow.
  • Submissions appear only for appointments created on or after April 9, 2025.
  • Default booking fields (Name, Email, Phone) don't show in the Forms tab — they already display in the Contact section. Default-form support is on the roadmap.

Why use it

You can review intake questions and custom data right before a meeting, skip the round-trip between modules, and walk into appointments already informed. That's it — no setup needed beyond attaching a custom form to the booking flow.
