How to Add Custom Forms to Calendars in Sparkforge
Custom forms on Sparkforge calendars run after a visitor picks a time slot but before the booking is confirmed. Attach a form under each calendar's Forms & Payment tab — the form must be created first under Sites > Forms.
A custom form on a calendar collects intake data — qualifying questions, consent, project details — after a visitor picks a time but before they confirm the booking. The data lands on the contact record and can trigger downstream automations.
Build the form first under Sites > Forms. Then attach it per calendar.
Why attach a form to a calendar
- Lead qualification — surface unqualified prospects before they consume a slot.
- Automated data collection — map responses to custom fields on the contact record.
- Compliance and consent — capture agreements and signatures at the booking step.
- Workflow triggers — fire automations off custom field responses.
- Less back-and-forth — show up to the call with the details you need.
Attach a custom form to a calendar
Step 1: open the calendar
- In the main menu, go to Calendars.
- Click Calendar Settings in the secondary navigation.
- Find the calendar and click Edit next to its name.
Step 2: select the form
- Open the Forms & Payment tab in the left navigation.
- Under the Select Form dropdown, choose your form.
- Click Save in the upper right.
Reuse the same form on multiple calendars
Forms are not auto-applied across calendars. If you want one intake form on every service calendar, repeat the steps above for each calendar — open the calendar settings, edit, choose the form, save.
That manual step is intentional: it lets you attach different forms to different booking flows when you need to.
